INTRODUCTION

Affordable Basic Care™ (ABC™) has been created to provide pre-paid access to a certain number of primary care health services. It was developed by physicians to provide affordable access to high quality basic primary care services without the added expense usually associated with traditional managed care programs.

Our focus is on those individuals who:
  • Do not have insurance
  • Are underinsured
  • Have an insurance plan that has a large annual deductible.
  • COBRA Option

This program provides affordability and access of quality basic care services in the privacy of a primary care physician's office. It offers plenty of accessibility via a large panel of participating primary care physicians from which to choose.

SERVICE AND EASE-OF-USE

•  No health questionnaires or pre-qualifying medical exams.
•  No age limit.

It is very easy to enroll in ABC.

  • Thoroughly read our Member Agreement with an authorized ABC Enrollment Specialist.
  • After reviewing the Member Agreement sign the signature page.
  • Fill out the Enrollment Form and select a participating Primary Care Physician from the ABC Physician List.
  • Send the signed Member Agreement, the completed Enrollment Form and your payment* to the ABC mailing address.
  • We will complete the enrollment process and send you your I.D. card and other important information regarding the program to the address listed on the enrollment form.

*Participants can pay the annual fee by check, VISA, MasterCard, Discover or American Express.

ABC is NOT insurance. The care you receive is limited to services provided by your selected PCP in exchange for your pre-paid fee as described in the Member Participation Agreement
