| ABOUT AFFORABLE BASIC CARE (ABC) |
| Affordable Basic Care (ABC) is a locally owned and operated program that provides a primary care medical home for those individuals who either do not have any form of health coverage or have coverage that does not provide for adequate primary care services. ABC is NOT insurance. It is intended to provide quality basic primary care services at an affordable price. |
| When you enroll in ABC, you must choose a participating Primary Care Physician (PCP) from the list. Your PCP will provide your basic primary care needs in their office. |

| YOUR IDENTIFICATION CARD |
| Your ABC identification card enables you to receive covered services from your PCP. Each enrolled member of ABC will receive a card in his or her name with his or her own identification number. You must have your card with you when you visit your PCP for care. |

| CHANGE IN PRIMARY CARE PHYSICIAN (PCP) |
| You can only change to another PCP if: |

| ABC is NOT insurance. The care you receive is limited to services provided by your selected PCP in exchange for your pre-paid fee as described in the Member Participation Agreement. |
