This guide is intended to assist you in understanding the basic primary care services provided under this program. It is important that you have a clear understanding of what services are provided for the annual fee that you invest.
We ask that you take a few minutes to better understand the program. It will help in reducing potential misunderstandings with your Primary Care Physician (PCP) in the future.

ABOUT AFFORABLE BASIC CARE (ABC)
Affordable Basic Care (ABC) is a locally owned and operated program that provides a primary care medical home for those individuals who either do not have any form of health coverage or have coverage that does not provide for adequate primary care services. ABC is NOT insurance. It is intended to provide quality basic primary care services at an affordable price.
When you enroll in ABC, you must choose a participating Primary Care Physician (PCP) from the list. Your PCP will provide your basic primary care needs in their office.
MEMBER ASSISTANCE
If you are not able to get answers to your questions from this guide, please do one of the following:
  1. Call Us: (616) 458-2025 between the hours of 9:00 a.m. and 4:30 p.m. Monday thru Friday. Please have your identification card ready when you call.

  3. Fax Us: (616) 458-7787. Always include your name and identification number on your fax. Also, please include a daytime number where you can be reached.

YOUR IDENTIFICATION CARD
Your ABC identification card enables you to receive covered services from your PCP. Each enrolled member of ABC will receive a card in his or her name with his or her own identification number. You must have your card with you when you visit your PCP for care.
  1. Be sure to keep your card with you at all times. You must present your card at your PCP office prior to receiving care. No Card..No Care.
     
  2. If you lose your card, call ABC at (616) 458-2025.
     
  3. Only the enrolled member can use their card. Do not give your card to anyone. When you go to your PCP office for care, you will be required to show your card along with a second piece of identification. This is for your protection should you lose your card.
CHANGE IN PRIMARY CARE PHYSICIAN (PCP)
You can only change to another PCP if:
  1. Your current PCP agrees to your request during the contract year.
     
  2. Your current PCP ceases to actively practice. You will only be entitled to the remaining number of visits per your contract.
     
  3. All requests to change PCP during the contract year MUST be approved by ABC.
REMEMBER...
ABC is NOT insurance. The care you receive is limited to services provided by your selected PCP in exchange for your pre-paid fee as described in the Member Participation Agreement.
